When a student transfers from one school to another, enters high school or is withdrawn for any reason, the parents/guardians must notify the school of the student’s new learning institution. The new school may request a copy of the permanent record and health card from the former school. Permanent records will not be released directly to parents/guardians. The school must obtain an Authorization to Release Records Request Form. All books and materials must be returned. All bills must be paid before the records are transferred to another school.
Please complete the following form to request your transcript/release of records. Parents or guardians must complete a separate form for each child if they have multiple children.
It's important to know that processing the request might take about a week. Signing the form confirms there are no unpaid fees or outstanding financial obligations — additionally, a fee of $10.00 for obtaining the transcript when the application is submitted.
